Did you recently register yourself and/or your spouse for benefits? This information is for you!

Please remember to arrive 10 minutes early to ensure a smooth check in, and allow extra time post screening to review your results and wellness plan with your wellness team.  

New to Caesars medical plan? Your Wellness Rewards requirements and deadlines depend on the date your benefits became effective. Check out Important Dates for an overview.

By participating, you can keep your medical premiums low by saving $1,300 a year ($2,600 if your covered spouse participates).

If you or your covered spouse do not complete the required activities by the deadlines listed, you will be subject to the $50 per paycheck ($25 if paid weekly) medical plan premium increase per deadline missed.

Condition Management and Bonus Reward

Participants may be eligible to earn a bonus payment into their HRA or HSA and/or complete the condition management program if biometric screening values meet the goals criteria by category, outlined in the Goals chart below. In order to qualify for the outcomes based bonus, participants must have a prior plan year biometric screening (completed before June 1st ) and complete current year biometric screening before November 30th. If you are new to Caesars benefits, bonus eligibility is based on your unique program deadlines. Consult with your WellNurse for bonus eligibility.

If you are enrolled in the HMO plan, you are not eligible to receive a bonus as your health plan does not come with an HRA or HSA.

If you have concerns about your ability to complete Wellness Rewards program activities or requirements listed above, please see your WellNurse or call 800-591-9220 to discuss alternatives. If your current circumstances will prevent you from achieving the results required for earning the wellness bonus, we will work with you to develop a wellness program that is best suited to those circumstances.
